What is PHN Stepped Care Referrals?
At Mindcare, we understand the financial barriers that prevent people from accessing professional therapy and psychology services. That’s why we are happy to be a preferred provider under the Primary Health Network’s (PHN) Stepped Care program. Stepped Care is an Australian Government Initiative that means clients don’t have to pay the financial gap between the bulk bill rate for Mental Health Care Plans and the service delivery cost. People with limited financial means are generally eligible to avoid the out-of-pocket expense.
Mindcare Mental Health Services is a team of experienced and committed psychologists offering assessments, psychology, NDIS services and specialised behavour support for a broad range of mental health and psychosocial needs.
Our centres are located across the Sunshine Coast, Noosa, Gympie and Moreton Bay regions. We also offer telehealth and in-home therapy for NDIS participants.
How do to check if you are eligible for financial assistance with Stepped Care?
Go to your local GP and ask for a Mental Health Care Plan.
Your GP will then email Mindcare your Mental Health Care Plan.
Mindcare MHS will complete a Stepped Care Referral form with your Mental Health Care Plan.
Mindcare MHS will submit your referral to Primary Health Network PHN.
If you are deemed eligible by the PHN you will receive financially aided psychology supports.
Mindcare MHS friendly coordinator will contact you to book your sessions at our centres or via telehealth.
Please note that PHN Stepped Care Referrals are only available in certain areas. To find out if your eligible, contact our Mindcare team with your location.
